At the same time, the disease is relatively private, and patients may avoid medical treatment or have no way to seek treatment. In order to count the disease prevalence in large population and provide medical channels for patients in need, Peking Union Medical College Hospital is currently carrying out research on semen allergic reaction. We believe that with the increase of medical cases, we will know more about this kind of rare diseases. Patients suffering from semen allergy are more likely to get effective diagnosis and treatment.
One morning in August 2020, I went out of the clinic with my tutor Professor Yin Jia in Peking Union Medical College Hospital. A 22-year-old boy from Peking University came to the clinic with a stack of sorted literature. After sitting down, he said to us: after consulting all the foreign literatures, I think I may be allergic to my own semen. I think I have “post orgasm syndrome”. He said that from puberty, within a few minutes after masturbation, there will be flu like symptoms such as sneezing, runny nose and stuffy nose; at the same time, there will also be mental symptoms such as fatigue, lack of concentration, memory loss and incoherence of language, which can last for seven days after ejaculation; in addition, he observed that he has symptoms in both masturbation and sexual life, but almost no symptoms in erection without ejaculation No discomfort.
He talked about his previous medical experience, and all hospital departments did not give a clear answer to the symptoms he described, but only diagnosed as allergic rhinitis, or thought that his masturbation had nothing to do with the related symptoms. After he searched the Internet for the first case report of “post orgasm syndrome” published by our research group, his hope revived. It is true that he is not the first patient with post orgasmic syndrome, but there are very few patients with this disease.
Post orgasmic syndrome (PIs) refers to men’s “allergy” to autologous semen. What is post orgasmic syndrome (POIs)? As a matter of fact, the understanding of it at home and abroad is just beginning, and more than 50 cases have been reported in recent ten years. POIs was first discovered by foreign scholars in 2002. The case report showed that two men developed a group of symptoms after ejaculation, such as extreme fatigue, muscle weakness, hot flashes and sweating, depression, memory loss, inattention, incoherence, nasal congestion, runny nose, sneezing, itching eyes, skin erythema, etc.
Professor Yin Jia found the first case of POIs in China in 2015 and published it in international journals. Therefore, if your male partner is a little “weak” after ejaculation, it may not be that he “can’t”, but that he is ill. How to judge if you have POIs?
At present, there are five common standards
(1) Have one or more of the following symptoms:
Influenza like symptoms include extreme fatigue, muscle weakness, hot flashes, sweating, depression, irritability, memory loss, inattention, incoherence, nasal congestion, runny nose and itchy eyes;
(2) All symptoms appeared within seconds to hours after ejaculation caused by sexual intercourse, masturbation or spermatorrhea;
(3) Symptoms appeared almost every time after ejaculation (probability > 90%);
(4) Most of the symptoms lasted for 2-7 days;
(5) The symptoms can be relieved by themselves.
Apart from physical discomfort, some people with POIs may have premature ejaculation. Moreover, due to a series of disturbing symptoms after ejaculation, most patients with POIs will consider reducing the frequency of ejaculation or sexual life, and also worry or feel guilty about maintaining the relationship between sexual partners. Young patients will adjust the time of their sex life to non study, work or social activities.
Therefore, POIs affects physical and mental health and quality of life in all aspects. Fortunately, there is no association between POIs and male infertility.
Why does ejaculation appear after a series of symptoms?
At present, the pathogenesis of post orgasmic syndrome is still under exploration, and it is likely that there are many causes leading to the syndrome. Researchers have made several conjectures. Some scholars believe that allergy plays a major role in POIs. They found that the skin test results of patients with POIs are almost all positive (positive skin test indicates allergic reaction). The male students from Peking University at the beginning of the article also have the same positive test results (see the figure below).

Symptoms can be explained as an immune response to the patient’s own semen. At the same time, POIs patients often suffer from other allergic diseases, such as allergic rhinitis, asthma, severe allergic reactions and so on. Three of the 4 patients with POIs were complicated with allergic rhinitis. However, our team found that both symptomatic and healthy men may have positive skin reactions to their own semen, and no specific indicators of semen allergy were detected in several patients. POIs can not fully explain it by allergy, and it is speculated that neuropsychological factors may be involved. Another possible mechanism is the abnormal testosterone level in patients, but there is little evidence to support it.
So if POIs is diagnosed, how to treat it?
At present, there is no unified treatment plan. Oral antihistamines, antipyretic, analgesic and anti-inflammatory drugs or psychotropic drugs may be effective for some patients, but they can not eliminate all the symptoms. Some foreign scholars have treated two patients with POIs by subcutaneous injection of autologous semen for 31 months and 15 months, and the symptoms were reduced by 60% and 90% respectively. The other two patients were relieved after injecting hCG and testosterone respectively.
Women are also allergic to semen
Post orgasmic syndrome is considered to be a rare disease, but more cases of female semen allergy have been reported. As early as 1958, foreign scholars reported the first case of female semen allergy. Since then, more and more cases have been found. Female semen allergy is a hypersensitivity reaction caused by sexual contact with semen. Semen is a kind of material with complex components. Besides sperm, it also contains many other components. After people eat or take medicine, food or medicine can also be stored in semen.

There have even been reports that a 43 year old Irish woman died after having sex with a shepherd dog. Doctors diagnosed her as having a severe allergic reaction. The allergen was the dog’s semen. In addition, similar to post orgasm syndrome, female patients with semen allergy are also prone to other allergic diseases.
How to judge if a woman has semen allergy?
First of all, it needs to be confirmed that this woman has allergic reaction at the contact site or the whole body after the male partner has sex without a condom. After using the condom, the symptoms of semen allergy can completely disappear. Here, we also need to pay attention to whether to wear the condom throughout the process, because some semen will flow out before ejaculation. Secondly, the need to exclude the semen contained in food, drugs or their own latex allergy. Of course, should also see a gynecological clinic, except for some gynecological diseases. In the aspect of examination, semen skin test can be carried out by preparing partner’s semen; blood can also be drawn to check whether there are semen specific antibodies in the blood.

What are the reasons for women’s allergy to male semen?
In fact, allergic to semen is not allergic to sperm, but other ingredients in semen. At present, it is believed that prostate-specific antigen is the most relevant allergen in patients with semen allergy, but other proteins may participate in it. Anaphylactic reaction refers to the reaction that occurs when an immune organism is stimulated by the same allergen again. However, nearly half of the women developed allergies when they first came into contact with semen, indicating that they had been sensitized before they first came into contact with semen.
Studies have shown that the female with first-time semen allergy may be related to contact with dogs. A protein secreted by the prostate in dog urine is highly similar to human prostate specific antigen and has cross reactivity. Some case reports have also mentioned that female patients with semen allergy have raised male dogs. But it’s not entirely true, because women who are allergic to semen are not allergic to dogs.
How to prevent and treat female semen allergy?
The most simple and effective way to prevent semen allergy is to use condom to avoid semen contact with vagina. For some patients with mild symptoms, oral antihistamines or antipyretic analgesics can improve symptoms. Desensitization therapy is also the treatment that most people want to accept. For women who need to be pregnant, they can gradually inject or drip diluted semen into the subcutaneous or vagina, and gradually increase the concentration until they adapt. It should be noted that after tolerating semen, we need to maintain a regular sexual life to maintain the curative effect. Of course, semen can also be treated as sperm for artificial insemination. At present, semen allergy does not seem to cause infertility. For fertility, the impact is more from women’s resistance to sexual behavior or the impact of allergic reaction on their own pregnancy.
For female patients with semen allergy, the simplest prevention method is to wear condoms throughout the process.
